• Someone who has a family member with an intellectual or developmental disability, who has the Public ​Guardian and Trustee as their substitute decision maker.
  
• Someone who has a friend with an intellectual or developmental disability, who has the Public Guardian and ​Trustee as their substitute decision maker.
  
• A CLdS funded agency or other service provider who works with someone with an intellectual or ​developmental disability, who has the Public Guardian and Trustee as their substitute decision maker.
  

What you can expect from filling out this referral form

• We will reach out to the person who filled out the form first, before talking to anyone else.
  
• If you are filling out this form for someone else, we will ask you questions to better understand the situation ​and what the best way to make contact with the person is. We will always get permission before talking to ​anyone else.
